*Do note that there will be an admin fee of $5 for Change of Class request regardless of class availability, hence kindly submit 'Change of Class Form' over the front counter. Thank you.
*Click on the to download the appropriate form. If a notice appears "The author would like you to open this as read-only, unless you need to make changes. Open as read-only?", select 'No' and fill in the shaded areas accordingly. Then e-mail it back to the appropriate e-mail (as above). Thank you!
